Getting the right care at home shouldn’t feel overwhelming. For those eligible for NHS Continuing Healthcare, we work closely with NHS teams and other care professionals to help provide personalised support at home — from short-term assistance to comprehensive, 24-hour care.

Once your care needs and funding have been agreed, we work alongside Continuing Healthcare teams, nurses, social workers and other professionals involved in your care to help put the right support in place. This joined-up approach helps ensure your care is well coordinated from the outset and continues to reflect your needs as they change.

Depending on your individual care plan, support may include personal care, medication assistance, companionship and more specialist help with complex health needs. Whether support is needed for a period of recovery or on an ongoing basis, care is shaped around your routines, preferences and the way you want to live.

Throughout, our focus is simple: to make receiving NHS-funded care at home feel coordinated, personal and reassuring — so you and those closest to you can feel confident in the support around you.
